cost - direct and indirect- include at start designing the system, training the managers, paperwork and so on. In between it all reduces organisation’s productivity and that is a cost too. Many organisations does not have this kind of resources and that is why many times this process is being done very quickly and as cheap as possible- this reduces the quality of the tool.
